CHAPTER 4 The Patient Named Silvana

Laura stared at the hospital wristband so long that the letters began to blur.

Patient: Silvana Armenta Ortiz
Admission: Wednesday, 8:00 a.m.
Procedure: Confidential

No department.

No doctor's name.

No description of the surgery.

Only a barcode and the logo of San Gabriel Medical Center, one of the most exclusive private hospitals in Cuernavaca.

The same hospital where Laura had undergone surgery three weeks earlier.

The same hospital where she had signed documents she never read.

The same hospital where the smiling doctor had assured her everything had gone perfectly.

Daniel tugged gently on her sleeve.

"Mom?"

Laura folded the wristband into her palm before he could read it.

"We're going home."

"But my ice cream..."

"We'll get another one tomorrow."

Daniel nodded without complaining.

Children always knew when something had changed.


The drive home felt endless.

Every red light became an interrogation.

Every car in the rearview mirror looked suspicious.

Twice she changed direction.

Three times she circled different blocks.

A black SUV stayed behind her for nearly ten minutes before finally turning away.

Maybe it was coincidence.

Maybe it wasn't.

She no longer believed in coincidences.


When they arrived home, Eduardo's car was already in the driveway.

Too early.

He normally didn't return until after six.

Laura looked at the dashboard clock.

4:12 p.m.

Something had happened.

She took a slow breath before opening the front door.

Eduardo stood in the kitchen preparing coffee.

He smiled exactly as he always did.

"There you are."

His voice was calm.

Too calm.

"I got off work early."

Laura forced herself to smile.

"Everything okay?"

"Actually..."

He placed two mugs on the counter.

"I have some wonderful news."

Her stomach tightened.

"What news?"

"My company wants to promote me."

He looked genuinely excited.

"A regional position."

"Congratulations."

"It comes with one condition."

There it was.

The trap.

Laura wrapped both hands around the warm coffee mug, hiding the slight tremble in her fingers.

"What condition?"

"They need both spouses to update some legal paperwork."

She didn't answer.

Eduardo continued.

"The meeting is tomorrow morning."

Tomorrow.

The same morning printed on Silvana's wristband.

The same morning the mysterious woman had warned her about.

Laura kept her expression neutral.

"What kind of paperwork?"

"Oh, nothing complicated."

He laughed softly.

"Just signatures."

There was that word again.

Signatures.

Always signatures.

Always papers she wasn't supposed to read.


That evening Laura called Elena from the laundry room while the washing machine ran.

"He wants me to sign documents tomorrow."

Elena didn't sound surprised.

"I expected that."

"I think it happens at the same time as Silvana's hospital procedure."

A long silence followed.

Finally Elena said,

"I've been digging into San Gabriel Medical Center."

"And?"

"The hospital has already been investigated twice."

Laura's pulse quickened.

"For what?"

"Illegal transplant allegations."

Laura leaned against the wall.

"What?"

"The investigations were closed."

"No evidence."

"No charges."

"But several doctors quietly resigned afterward."

Laura slowly slid down until she was sitting on the floor.

"This isn't about money anymore."

"No."

Elena's voice became very quiet.

"I don't think it ever was."


That night Laura couldn't sleep.

At 1:18 a.m., she heard Eduardo moving downstairs.

She waited.

The bedroom door closed softly.

A minute later, she followed.

She moved silently across the dark hallway until she reached his office.

Light spilled beneath the door.

Voices.

Eduardo wasn't alone.

Laura looked through the narrow gap.

A man sat across from him.

Gray hair.

Expensive suit.

Leather briefcase.

Not the doctor.

Not someone she'd ever seen before.

The stranger slid a thick folder across the desk.

Eduardo opened it.

Laura couldn't hear every word.

Only fragments.

"...risk..."

"...insurance..."

"...after tomorrow..."

Then the stranger spoke clearly enough for one sentence to reach her.

"If she refuses, the backup plan begins."

Laura's heartbeat pounded in her ears.

Eduardo nodded once.

"I understand."

The man stood.

As he walked toward the door, Laura barely had time to slip into the hallway closet.

She held her breath.

The stranger walked past.

Paused.

Laura froze.

The doorknob of the closet moved slightly.

Then footsteps continued toward the front door.

Only after the engine of a departing car faded into the distance did she dare move.


The next morning Eduardo was unusually cheerful.

He cooked breakfast.

Made pancakes for Daniel.

Poured fresh orange juice.

Even sang softly while washing dishes.

It felt like watching an actor perform happiness.

At 7:15 a.m., he looked at Laura.

"We should leave soon."

She smiled.

"Of course."

Exactly as the woman in the red coat had instructed.

Say yes.

Laura had no intention of signing anything.

But Eduardo didn't need to know that.


Instead of driving toward his office, Eduardo headed south.

Toward San Gabriel Medical Center.

Laura pretended not to notice.

"I thought the meeting was downtown."

"It changed."

"When?"

"This morning."

His answer came too quickly.

The hospital appeared ahead.

White buildings.

Glass walls.

Perfect landscaping.

Beautiful enough to hide terrible things.

Eduardo parked in the underground garage.

As they stepped from the elevator, Laura noticed something strange.

No one greeted Eduardo as a visitor.

Instead...

Several staff members nodded respectfully.

One nurse even smiled.

"Good morning, Mr. Ríos."

Not "sir."

Not "Mr. Eduardo."

She knew him.

Well.

Far too well.


Inside a private conference room sat three people.

A lawyer.

The familiar doctor.

And...

Silvana.

Laura's breath caught.

She had imagined meeting the other woman dozens of times.

Angry.

Elegant.

Cruel.

Instead, Silvana looked exhausted.

Her skin was pale.

Dark circles surrounded her eyes.

She wore ordinary clothes, but the hospital wristband circled her left wrist.

She wasn't pretending to be a patient.

She was one.

Silvana looked at Laura only briefly.

Then lowered her eyes.

Not arrogance.

Guilt.

Or fear.

The lawyer smiled professionally.

"Mrs. Mendoza."

"Please have a seat."

A thick stack of documents waited on the polished table.

Eduardo squeezed Laura's shoulder affectionately.

"See?"

"I told you."

"Just paperwork."

The lawyer slid the first page forward.

"Sign here."

Laura picked up the pen.

Her heart pounded so loudly she could barely hear the room around her.

She remembered the warning.

Say yes.

She smiled politely.

"May I read everything first?"

The lawyer hesitated.

Eduardo's smile stiffened.

"It's standard language."

"I always read before signing."

The room grew noticeably colder.

The lawyer cleared his throat.

"Of course."

He handed her the documents.

Laura began turning pages slowly.

One.

Two.

Three.

Ordinary legal language.

Then page seven.

Her fingers stopped.

A single paragraph had been highlighted in yellow.

In the event of unforeseen medical complications affecting Laura Mendoza, all previously executed authorizations shall remain legally binding and irreversible.

Medical complications.

Again.

Laura looked up.

The doctor was watching her.

Not the papers.

Her.

Studying every expression on her face.

As if waiting to see whether she understood.

She looked back down.

Page eight.

Page nine.

Then...

A folded sheet slipped from between the pages and landed silently in her lap.

No one else seemed to notice.

She unfolded it beneath the table.

Only six handwritten words appeared.

DON'T DRINK THE WATER. LEAVE NOW.

Laura's pulse exploded.

Almost at the same instant, a nurse entered carrying a silver tray.

Four glasses of water.

She placed one in front of each person.

The doctor smiled warmly.

"You must be thirsty after the drive."

Laura looked around the table.

Eduardo picked up his glass.

The lawyer picked up his.

Silvana...

Didn't touch hers.

Instead, she looked directly at Laura.

Very slightly...

Almost invisibly...
